This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above when not all staff members had complete personnel files which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher-Child Ratio (a) There shall be a ratio of 1 teacher visually observing and supervising no more than 12 children in attendance... This regulation was not met as evidenced by 15 preschool aged children being supervised by 1 staff on 05/20/2025. (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement was not met as evidenced by a staff member's visual observation being on diapering and not the children within the classroom, and biting incidents occurred when the staff member(s) could not say who bite; this poses/posed an immediate health, safety or personal rights risk to persons in care.

Reporting Requirements (d) Upon the occurrence,... a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days... (1)...(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met as evidenced by an unusual incident occurring on 06/13/2024 which was not reported to the Department.
