Buildings and Grounds (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement was not met as evidenced by LPA observing accessible cleaners and unlocked child locks in the Discovery Preschool classroom on 03/05/2026.
Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by staff not preventing children in care from engaging in harmful behaviors that resulted in injuries.
Outdoor Activity Space (e) As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls. This requirement is not met as evidenced by: Based on LPA observation, the licensee did not comply with the section cited above when the preschool playground has an uneven surface with cracks and a hard surface under the play structures which poses/posed a potential health, safety or personal rights risk to persons in care.
87355 Criminal Record Clearance (e) All individuals subject to criminal record review...shall prior to working...in a licensed facility: (1) obtain a California clearance...as required by the Department. This requirement is not met as evidenced by: Based on observation and record review, the Director did not ensure a criminal clearance was obtained for 1 staff member which poses an immediate health, safety and or personal rights risk to chidlren in care.
