Teacher-Child Ratio (a) There shall be a ratio one one teacher visually observing aldn supervising no more than 12 children in attendence... This requirement was not met as evidence by: Based on LPA's observation and interviews conducted, S2 went inside the facility to use the restroom and ended up helping a child throwing up, while S1 was left alone out of ratio outside. S1 was supervising 16 children. This is a potential risk to the health and safety of children in care.

Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
