No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced: Child #1 exited the classroom without staffs knowledge. Child exited the classroom into a fenced outdoor play area and reentered the classroom approximately 27 seconds later. This is an immediate risk to the health and safety of children in care. A civil penalty in the amount of $500.00 is assessed.

This requirement was not met as evidenced by: Child #1, who is allergic to dairy, was served food containing dairy on July 1, 2022 which caused the child to have an allergic reaction. Staff were aware of the child's food allergies and needs; however, on this day they were short staffed and they inadvertently served the food to the child.
