At 10:10 AM, LPA reviewed sleep log and observed that the log had not been filled out for 8/28/25. C1 woke up at 10:30 AM. Licensee confirmed that she had not completed the log today. LPA provided PIN 20-24-CCP-SP.

This requirement is not met as evidenced by: Based on observation, LPA observed a chlorine gallon was on a lower shelf in the laundry room; one has to pass the kitchen to access the laundry room. The kitchen is off-limits and has a safety gate but, it was not locked when LPA arrived. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
