Operation of A Family Child Care Home (g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: (3) Where children are less than five years old are in care, stairs shall be fenced or barricaded. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above inthat the stairs were not barricaded when LPA arrived which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above in that licensee was using garage for childcare which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep The provider shall supervise infants while they are sleeping and adhere to the following requirements: The provider shall physically check on the infant every 15 minutes. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above for C2. The sleep log for C2 indicates a last entry as June 3, 2025 however C2 has been in care following that day and there are no additional entries. This poses a potential health, safety or personal rights risk to children in care.
Operation of A Family Child Care Home (g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: (1) Fireplaces and open face heaters shall be screened to prevent access by children. The home shall contain a fire extinguisher and smoke detector device which meet standards established by the State Fire Marshall. This requirement is not met as evidenced by: Based on LPA Wilburn's observation, the licensee did not comply with the section cited above in the Fire Extinguisher was observed to not be Full and in the "Recharge" Area, which poses/posed a potential health, safety or personal rights risk to persons in care.
