Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review a current Mandated Reporter training certificate for S1, S2 and S4.

This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above in the facility uses tap water for food preparation and to water vegetable garden, offered to clients for consumption; and did not retest water outlets, after receiving a lead test report on 12/10/2019 documented results of a lead exceedance on an outlet in a lunch preparation area of 42.0 ppb., which poses a potential health, safety or personal rights risk to persons in care.
