(4) Mattresses shall be made specifically for the size crib or play yard in which they are placed. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. A child was laying on a long mattress that was placed inside of the play yard; the mattress is not the right size and bent upwards near the top of the play yard.
