This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above. In reviewing facility records LPA observed that child #1 had not been signed in since 7/11/2025 and was present during today's inspection. This poses/posed a potential health, safety or personal rights risk to persons in care.
