- Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. Documentation shall include the following: This requirement is not met as evidenced by: Based on the file review the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During the file review LPA was unable to review current sleep logs for C3. Additionally, the sleep log times are set times (i.e 12:00PM, 12:15PM, 12:30PM, 3:30PM, 3:45PM,). Licensee understands the times must reflect the actual time the child goes to sleeps.

During the facility tour LPA observed C4 being placed in the playpen upon arrival. As the tour continued LPA observed C4 awake with a blanket over him/her.
