This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above. In conducting a physical inspection and in reviewing the facility sketch, LPA observed a middle classroom that was created by taking away space/square footage from the infant classroom. The Department did not receive updated documentation an/or authorized the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. In touring the facility playground's, LPAs observed gaps and cracks on the rubber matting that is underneath the play structures. The gaps and cracks create a tripping hazard. This poses an immediate health, safety or personal rights risk to persons in care. *Please note that this is a repeat violation within a 12 month period and a $250.00 civil penalty is being issued*
