This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2 was missing proof of an updated Infant Needs and Services Plan as the latest one on file was dated for 08/09/2024 which poses a potential health, safety or personal rights risk to persons in care.

(c)(1) A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation... (2) Upon enrollment... the licensee shall provide to the parents or legal guardians of the newly enrolling child copies of any licensing report that the licensee has received during the prior 12-month period that documents any Type A citation... (3) The licensee shall require each recipient of the licensing report... to sign a statement indicating that he or she has received the document and the date it was received. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C5 and C6 were missing proof of the completed Acknowledgment of Receipt of Licensing Reports (LIC9224) which poses a potential health, safety or personal rights risk to persons in care.
(1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as it was reported that a daycare child walked out of the classroom without the classroom staff members seeing which posed an immediate health, safety or personal rights risk to persons in care.
