(b) Cribs or play yards shall be free from all loose articles and objects. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above as one infant child (Child 1), was observed sleeping in the crib with a pillow, which poses a potential health, safety or personal rights risk to persons in care.
Operation of A Family Child Care Home (a) The licensee shall be present in the home and shall ensure that children in care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. This requirement is not met as evidenced by: Based on LPA's inspection and record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. When LPA arrived at 12:00 pm, assistant Ziba Zendehdel stated that licensee Azita Khodaie had been at a doctor's appointment since 11:15 am. Licensee had not returned to the home by the end of LPA inspection, which is past 20% of licencee's operating hours (7:00am-5:30pm) which were established on 07/12/2018.

General Provisions and Definitions (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as both assistants did not have proof of immunization fro influenza, pertussis, and measles. Assistants were also missing other required from form LIC 126 which poses/posed a potential health, safety or personal rights risk to persons in care.
