Teacher Aide Qualifications:An aide shall work only under the direct supervision of a teacher. This requirement was not met as evidenced by 2 aides left supervising 5 preschool children with no supervision of a teacher which is an immediate health and safety concern to the children in care.
Child's Records: A separate, complete and current record for each child is maintained in the child care center. This requirement was not met as evidenced by: Based on interviews with the director there was some children that did not have files at the facility, including their emergency contact information. This poses a potential health, safety, or personal rights risk to persons in care.
