Responsibility for Providing Care and Supervision. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by an Unusual Incident Report received on 01/20/202. Facility self-reported that on 01/16/2026 Child 1 and Child 2 eloped from the facility unsupervised. The children were found at the residence of Child 1 approximately 30 minutes after the facility was aware the children were not present. This posed an immediate risk to the health and safety of children in care.
