(a)The licensee shall ensure that each child is accorded the following personal rights:(3)To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... This requirement was not met as evidenced by admission and documentation of an incident on 11/07/2025, where Staff 1 was observed physically forcing Child 1 on their nap cot and yelling at them. This is an immediate risk to the health and safety of children in care if not corrected.

This requirement was not based on a self-report from the facility that Child 1 was on the cheek while a stafff member had their dog at the facility. It was stated that the dog breed was a mini Ausie Shephard Bordie Collie. This is considered an immeidate risk to the health and safety of children of care.

Teacher-Child Ratio (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement was not met as evidenced by staff interviews and LPA observations of CSRs. Evidence concluded that the Transitions classroom was over ratio on 1/18/2024, when a staff was supervising 18 children by themselves. Evidence was sufficient to support that this has occurred in the past. If not corrected, this is an immediate risk to the health and safety of children in care.
