Personnel Records (a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, 3 of 6 Staff are missing the Mandated Reporter Certificate, and 1 of 6 Staff is missing the immunization record, which posesd a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties (b) Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below: (1) A teacher shall have completed, with passing grades, at least six postsecondary semester or equivalent quarter units of the education requirement specified in (c)(1) below; or shall have obtained a Child Development Assistant Permit issued by the California Commission on Teacher Credentialing. This requirement is not met as evidenced by: Based on observation, interview and records reviewed, the licensee did not comply with the section cited above in 3 of 3 teacher's assistants or aide are left with the children. No fully qualified teacher present at the time of inspection, which poses a potential health, safety or personal rights risk to persons in care.
