This requirement is not met as evidenced by: Based on LPA’s interviews, 5 out of 8 staff stated the previous director did not report to parents and to Licensing of a Hand Foot Mouth (HFM) outbreak. LPA reviewed the file and confirmed facility did not notify the Dept. about the HFM outbreak from last year.
