No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This was not met as evidenced by: Based on interview and review of records of the unusual incident on 1/20/22, it was determined that a child #1 wandered away from the supervision of staff, and was undupervised for approximately 55 minutes at a play structure nearby the campus of Culverdale Elementary. This structure is approximately 1000 feet from the licensed facility. This is an immeidate threat to the child's health, safety and personal rights.
