Teacher-Child Ratio 101216.3(a): (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance. This requirement was not met as evidenced by: Based on staff interviews & record review, S2, and S4 disclosed out of ratio. Records show on 03/11/26 there were 13 preschool children with 1 staff for about 2 minutes. S4 disclosed walking out of the classroom often to make copies or for a bathroom break leaving S2 alone with the children. This poses an immediate health and safety risk to the children in care.
