- Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on observation, iinterview and record review, the licensee did not comply with the section cited above in 3 out of 3 staff did not have a valid Mandated Reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by interviewing staff, and reviewing the documents in the Department's computer system that the facility had Covid tested positive for two staff and one day care child. Licensee failed to report the Covid cases to the Public Local Health Department and our Department as it was required by Regulations. This is a potential risk to the health and safety of children in care.

When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult...................Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. This requirement was not met as evidenced by interviewing 7 adults and 5 parents and Licensee's admission having outside part time employment. - A large family day care home may provide care for more than 12 children and up to and including 14 children, if all of the following conditions are met: At least one child is enrolled in and attending kindergarten or elementary school and a second child is at least six years of age. This requirement was not met as evidenced by LPA's observation and reviewing birth dates of children on 2-22-2022 that there were 14 children present with 4 staff including 2 children under 2 years old and 12 children ages 2 to 4 years old. The facility failed to meet the requirement to include two school age children in the group of 14 children, This is a potential hazard to the health and safety
