This requirement is not met as evidenced by: Based on observation today, the licensee did not comply with the section cited above in one count out of one count that the sandbox contained dirt and not enough sand and the sandbox contained foreign materials in it which poses a potential health, and safety risk to persons in care.

General Provisions and Definitions (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on the record reviewing today, the licensee did not comply with the section cited above in 4 counts out of 8 files that staff # 3, 4, 5, and 7 did not have the required immunization such as MMR and Pertussis in their files which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants- (a) In addition to Section 101229, the following shall apply:(1)Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as evidenced by a self report incident, 3 staffs' interviews, and reviewing documents that S1 left two 10 months old children unattended in the room for approximately 3 to 4 minutes in order to get a buggy out of the storage room away from the room during transitioning children to playground. This is an immediate risk to the health and safety of children in care.
