Report finding: 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time…This requirement is not met as evidenced by: Based on the information gathered from the unusual incident report, interviews with two staff, and review statements from the four staff. The children were in the bathroom together for approximately five minutes without the supervision. This is an immediately safety risk to children in care.

Report finding: Lead Testing (1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test. This requirement is not met as evidenced by: Deficient Practice Statement Based on interview with Director and record review, the licensee did not comply with the section cited above as 2 out of 2 outlets have not been tested for lead exceedance. Direcror was unable to provide LPA with test results of outlets in Room #Y1 which poses a potential health, safety or personal rights risk to persons in care.
