This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above.While touring the inside activity space at 9:13 AM, LPA observed an unlocked cabinet in the children's restroom inside classroom #7 with seven children present. Inside the unlocked cabinet, LPA observed Lysol All Purpose Cleaner that reads "Keep Out of Reach of Children." The facility is being assessed a civil penal for the amount of $250.00 for a repeat violation cited on 1/12/2024. which poses an immediate health, safety or personal rights risk to persons in care.

- Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on LPA's reviewing files for staff today at 11:00 am, LPA noticed among 14 staffs' files which were reviewed 6 staffs' Mandated Reporter Training Certificates were expired on their files. The licensee did not comply with the section cited above which poses a potential health, and safety risk to persons in care.
