Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in Staff#2 did not have record of their immunization in file which poses/posed a potential health, safety or personal rights risk to persons in care.

101700.3 California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidenced by: Based on facility Lead sampling tests it was discovered that the drinking fountain outlets in Rm 2, Rm 3, Rm 5, and Rm 6 had high level of lead.
