(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This was not met as evidenced by: Based on LPA observation and interviews, on 3/11/2026 staff did not have visual supervision of C1 and C2 resulting in C2 pushing over a bookshelf on to C1 which poses a potential risk to the health, safety, and personal rights of persons in care.

The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by: Based on corroborating information from staff members interviewed and staff S2's own admission on 02/14/23, S2 violated C1's personal rights by speaking to C1 inappropriately and yelling at the child . This poses a potential health and safety risk for the children in care.
