This was not met as evidenced by: Based on adult interviews, on 11/4/25 the licensee used the indoor play area for a presentation in which all preschool children attended. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care.

(1)The licensee shall provide a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities. D1 stated she doesn't agree. D1 will create an inventory list of toys with a clear benchmark for the # of toys that will be in preschool yard and provide a copy to jennifer.patel@dss.ca.gov. D1 will ensure broken or mishapen toys are removed and replaced. This was not met as evidenced by: Based on statements provided by adult interviews and LPAs observations revealed limited availabilty of children's toys, specfically in the outdoor play yard. Additionally, LPAs observed toys and equipment that were rusted, in disrepair, and contained sharp edges. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care. D1 will continue their plan of the opener does a playground walkthough and the closer does a playground walkthrough.

Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review at 11:00am, on 2/16/22, the licensee did not comply with the section cited above as three staff files did not contain current Mandated Reporter certificates as they had expired which poses a potential health, safety or personal rights risk to persons in care.
