This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in one out of three staff (LS, S1-S2). S2s file did not include proof of immunity to Pertussis, Measles, Influenza, or a negative TB test which poses a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in two out of five (C2 & C4) children's files. The licensee stated she was unaware of this form; therefore did not include these forms in the children's files. This poses a potential health, safety or personal rights risk to persons in care.

Documentation shall include the following: Time of each 15-minute check This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in two out of five (C2 & C4) children's files. The licensee stated she was unaware of documenting 15 minute checks on children 0-24 months old. Licensee did have a sleep log documenting name, age, dates. The only time documented was the time the child was laid down for sleep. This poses a potential health, safety or personal rights risk to persons in care.
