(1) No child(ren) shall be left without the supervision of a teacher … Supervision shall include visual observation. This requirement was not met as evidenced by: On 10/20/25, while at the facility, a child was bitten by another child. No staff member observed how the child sustained the bite or could explain the events leading to the injury, and visual observation of the child was not maintained at the time of the incident.
