- Infant Safe Sleep An infant shall not be swaddled while in care. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. LPA observed Child #5 (C5) was wrapped with a blanket and had a pacifier on her mouth while she was lay inside the play yard. Licensee immediately removed the blanket and the pacifier from C5. Licensee stated that it was a miscommunication between her and Staff #2 (S2). This poses a potential health, safety, or personal rights risk to children in care.
