This requirement is not met as evidenced by: Based on record review, the licensee has not provided proof to the Department that this facility has a qualified director/site supervisor on site. Reporting Requirements: (d) Upon the occurrence...a report shall be made to the Department by telephone or fax within the Department's next working day... In addition, a written report...shall be submitted to the Department within seven days following the occurrence...(1)Events reported shall include the following: (E) Epidemic outbreaks. This requirement is not met as evidenced by: Based on record review, LPA observed four children were reported absent on sign-in and sign-out. Per staff, three children reported absent due to cold and one child absent due to fever. Based on record review and interview, facility representative did not report a possible epidemic outbreak to Community Care Licensing (CCL) as required. This poses a potential risk to the health, safety, and personal rights of children in care.
