Supervision shall include visual observation.This requirement was not met as evidenced by: Information received through video surveillance obtained from director. It indicated children were left unsupervised in the school aged classroom for more than 3 minutes. This poses an immediate risk to the health, safety or personal rights of children in care. An immediate $500 civil penalty is being assessed.
