Health-Related Services (3) Prescription medications may be administered if all of the following conditions are met: (A) Prescription medications shall be administered in accordance with the label directions as prescribed by the child's physician. This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above in two children's medications (epi-pens) at the facility that were expired, which poses/posed a potential health, safety or personal rights risk to persons in care.
Facility shall report to the Department by telephone or fax within the Departments next working day and during its normal business hours...Any injury to a child that requires medical treatment This requirement is not met as evidence by...based upon interviews and documentation recieved, the facility did not report the incident within 24hours after the incident occured. This poses a potential risk to health, safety and/or personal rights to the children in care.
Discipline (a) Any form of discipline or punishment that violates a child's personal rights as specified in Section 101223 shall not be permitted regardless of authorized representative consent or authorization. This requirement was not met as evidence by: interviews conducted of an incident occuring on 12/19/2024 of a child climbing on classroom furniture and staff member pulled child by the back of the shirt to get them down. This poses a potential risk to the health, safety, and personal rights of children in care.
Teacher-Child Ratio ( a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement was not met as evidence by: Based on interviews conducted staff to children ratio has gone over in the mornings for 5 to 20 minutes at least 2 to 3 children and at least 3 times a week,This poses a potential health and safety risk to children in care.
