CONDUCT INIMICAL: Conduct in the operation or maintenance of a day care which is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility... This requirement was not met as evidence by: Based on interviews, S1 corroberated that the dates provided for the disaster drill were filled out on LPAs annual inspection date and were not the dates the drills were conducted as S1, forgot to document in the drill log. S1 stated RP, provided the dates. This poses a potential health and safety risk to children in care.
No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by the fact that on 05/03/2023 the facility was not aware that Child#1 and Child#2 wandered away from the facility until a neighbor escorted them back to the facility. This was an immediete risk to children in care.

The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by the fact that on 05/03/2023 two children were able to unlock the door of the main entrance of the facility and wander away before being brought back by a neighbor. This was an immediete risk to children in care.
