Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidenced by: Based on observation, the licensee did comply with the section cited above in 1 out of 2 toilets in the Oak Classroom was flooding but was fixed prior to LPA leaving. The Oak classroom has sufficient toilets, urinals and hand washing sinks for children in the classroom excluding the flooded toilet.

Health-Health Services. Prescription medications shall be administered in accordance with the label directions as prescribed by the child's physician. This requirement was not met as evidenced by: LPA observed Child #1's Epinephrine Injection did not come with the label direction as prescribed by physician and expired on May 2021.
