Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time. Supervision shall include visual observation. This requirement is not met as evidenced by: LPAs own observation, and staff and children interviews of children walking to the bathroom alone without staff and children also falling in the bathroom as they are also alone in the bathroom. This poses an immediate health and safety risk to children in care.
