Personal Rights Each authorized representative shall be asked to sign and date the acknowledgement-of-receipt statement at the bottom of the LIC 613A (9/96). This documentation shall be kept in the child's file. This requirement is not met as evidenced by LPA observing that the LIC613A: Personal Rights forms are missing signatures from parents/authorized representatives. This poses a potential health and safety risk to chidlren in care.
