Outdoor Activity Space: (c) Equipment and activity areas shall be arranged so that there is no hazard from conflicting activities. (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced by: Interviews & photos, tree branches were low and child was scratched and cat feces observed in the outdoor play yard and the child had cat feces on clothes and shoes. This poses a potential health and safety risk to children in care.
