Responsibility for Providing Care and Supervision for Infants (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement is not met as evidenced by: Based on record review and interview, licensee did not ensure that CHild #1 was provided visual supervision at all times, resulting in the child being left unattended on the outdoor play area. This was an immediate risk to the health, safety, and personal rights of child in care.
