(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive .... an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: On 10/17/25 visit staff assistant #1 did not have immunization records. On 10/22/25, immunizatio records only shows TDap, remaining are still pending to be provided and not aviable during visit date of 12/4/25.
Operation of A Family Child Care Home (g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: (1) Fireplaces and open face heaters shall be screened to prevent access by children. The home shall contain a fire extinguisher and smoke detector device which meet standards established by the State Fire Marshall. This requirement is not met as evidenced by: Based on At 10:12am Fire extinguisher was inspected by LPA Calderon and receipt for date of purchase was observed dated 11/05/2024, picture taken , the licensee did not comply with the section cited above in [1] out of [1] fire exting. which poses a potential health, safety or personal rights risk to persons in care.
