This requirement is not met as evidenced by: Based on observation and ecord review, the licensee did not comply with the section cited above in 12 counts out of 103 counts which poses/posed a potential health, safety or personal rights risk to persons in care. PER LPA OBSERVATIONS 103 CHILDREN WHERE PRESENT AND ONLY 91 CHILDREN WERE SIGNED IN ON THE SIGN IN/OUT SHEETS. 12 CHILDREN WERE NOT SIGNED IN.
