Reporting Requirements: Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours... Personal Rights: Each child receiving services from a family child care home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized... To receive safe, healthful and comfortable accommodations, furnishings... This requirement was not met as evidence by: Based on interviews conducted it was disclosed by S1-S3 and C1-C2 that S1 and S4 were involved in a verbal altercation in front of children. This poses a potential health and safety risk to children in care.
Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidenced by: Based on observations the licensee did not comply ,with the section cited above . During the tour LPA observed black, brown , yellow stains and holes on the carpet . Water dispencer had stains and dust . LPA observed couch cushions soiled , which poses/posed a potential health, safety or personal rights risk to persons in care.
