Cribs shall be free from all loose articles and objects, including blankets and pillows. This requirement is not met as evidenced by: Based on LPA observations, 3 out of 3 infants napping in cribs with blanket over their body as they napped (photo taken). This poses a potential risk to the health and safety of children in care.

Documentation shall include the following: (3) Time of each 15-minute check This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above. LPAs asked to see the 15 minute sleep log. Both staff and administrator stated that they thought that the sleep log was only for children under 12 months which poses a potential health, safety or personal rights risk to persons in care.
