General Provisions and Definitions (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on records review and interview, the licensee did not comply with the section cited above. During the staff file review, LPA observed S2, S4, S7, S8, S9 are missing proof of TDap, S3, S8, S9 are missing proof of measles vaccine, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on staff interviews and video footage watched from 03/01/2022, child#1 fell down and child#1's right hand and right side of face touched the cement ground at 9:55 a.m. on while on the back outdoor playarea and cried and teachers did not observe it. This poses a potential health and safety risk to the children in care.
