Additionally, LPA observed two dead roaches in the kitchen cabinet and one in the cabinet under the sink located in the 2A classroom. Cocroach traps and baking soda observed in the cabinets.

This requirement is not met as evidenced by: Based on observations, records review and interview , the licensee did not comply with the section cited above. 6 out 9 staff files were missing Mandated Reporter Training , which poses/posed a potential health, safety or personal rights risk to persons in care.
