Responsibility for providing care and supervision for infants Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not met as evidenced by... Based on LPA's observation on 03/16/26, an infant was napping in nap room with door closed and no staff in the nap room. Per staff they keep the nap room door closed and check on napping infant every 15 minuties. Napping infants were not under direct supervison while napping in the nap room. This poses an immediate health, safety, and personal right risk to the children in care.
