Child 5 lifted child1 and throw to the side, which was witneseed by child 3 and child4. This poses potential Health and Safety risk to the children in care.

The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. this requirement was not met as evidenced by Site Director will provide LPA a statement documenting that staff will enforce and have children follow the Play smart rules of the playstructure for the children's safety. Based on observations and interviews, the staff did not ensure that the child was following the play smart rules that are posted at the play structure to ensure proper use as instructed by the play structure manufacture.
