Responsibility for Providing Care and Supervision. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Based on interviews, the facility did not comply with the section cited above in child being left without the supervision of a teacher which poses a potential health, safety or personal rights risk to persons in care..
Reporting Requirements:101212(d)(1)(b) Upon the occurrence...of any of the events specified.. a report shall be made to the Department by telephone or fax..Any injury to any child that requires medical treatment. This requirement was not met as evidenced by: It was revealed that the facility did not follow up with Parent #1 to confirm receipt of a message sent through the Pro Care app at approximately 10:00 AM, when Child #1 was initially injured. Parent #1 became aware of the message only after calling the facility regarding a separate issue. This delay in communication may pose a potential health and safety risk to children in care.
