§1596.841 Current roster of children provided care in facility required Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request. This requirement is not met as evidenced by: Based on observation,interview, and record review, the licensee did not comply with the section cited above in there were seven children missing their address and physican's name and phone numbers and the two infants in care were not added to the child/parent roster which poses a potential health, safety or personal rights risk to persons in care.

Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on observation, interview, and record review, the licensee did not comply with the section cited above by the Director not having completed the correct Mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.
