(1) No child(ren) shall be left without the supervision of a teacher...Supervision shall include visual observation. This requirement is not met as evidence by, per LPA Laureano's reviewed of the Unusual Incident Report received, and interviews completed, it is confirmed that facility left child without visual supervision on 4/30/2025 for approximately 10 minutes, which poses an immediate health and safety risks to persons in care.
